Medical Liability and Information Forms
For the safety and protection of your teenager and our ministry, we ask
that all students keep the following forms on file with the youth
ministry office. Please print each one out and turn them in to Pastor
Chris Ziegler.

Volunteer Application
Packet
All adult volunteers who work with the Net Student Ministry are required
to complete an application process. The application packet will
explain the process and tell you how to begin. Currently we are
not able to accept online applications, so you must print the packet out
to complete it.
